CORNERSTONE PEDIATRICS VACCINATION SCHEDULE

@   2 WEEKS:  1 INJECTION

• Hepatitis B if not given at birth

@   2 MONTHS: 5 INJECTIONS, 1 ORAL

• DTaP (diptheria, tetanus, pertussis), HIB, Prevnar 13, Rotateq, Polio, Hepatitis B

@  4 MONTHS: 5 INJECTIONS

• DTaP (diptheria, tetanus, pertussis), HIB, Prevnar 13, Rotateq, Polio

@  6 MONTHS: 4 INJECTIONS

• DTaP (diptheria, tetanus, pertussis), HIB, Prevnar 13, Rotateq

@  9 MONTHS: 2 INJECTIONS

• Polio, Hepatitis B

@  12 MONTHS: 3 INJECTIONS

• Varicella (chicken pox), Hepatitis A, MMR (measles, mumps, rubella)

@  15 MONTHS: 2 INJECTIONS

• HIB, Prevnar 13

@  18 MONTHS: 1 INJECTION

• DTaP (diptheria, tetanus, pertussis)

@  2 YEARS: 1  INJECTION

• Hepatitis A

@  4 YEARS: 4  INJECTIONS/ HEARING & VISION TEST

• DTaP (diptheria, tetanus, pertussis), Polio, MMR (measles, mumps, rubella), Varicella (chicken pox)

@ 11 YEARS: 3  INJECTIONS

• Adacel (tetanus,pertussis), Meningitis, HPV (human papillomavirus)

@ 16 YEARS: 2  INJECTIONS

• Meningitis (Menactra , Trumenba)

**Hearing & Vision Screening begins at age 4 years
    and repeated every other year
